Non-surgical treatment fails when the piles are too large to respond, when they prolapse and cannot be manually reduced, when bleeding is heavy and recurring, or when the patient has tried conservative management for 3 to 6 months without meaningful improvement.
In these cases, continuing non-surgical treatment is not conservative — it is simply delayed treatment. The piles will not improve on their own at Grade 3 or 4.
