What Are Piles?
Piles — clinically called haemorrhoids — are enlarged and engorged blood vessels in and around the anal canal. They form when the vascular cushions that line the lower rectum become swollen due to increased pressure, straining, chronic constipation, prolonged sitting, low-fibre diet, pregnancy, or simple genetic predisposition. In my OPD I see patients of every age — college students, mothers, drivers, office workers, retired professionals.
In my experience, piles are the most common reason patients in Karachi walk into a proctology clinic — and also the most commonly misdiagnosed. A surprising number of patients I see have actually been told they have piles when the real problem is a fissure or an early fistula. That is why I always start with a proper proctoscopy examination, not just symptoms.
Internal piles develop inside the anal canal and are graded 1 to 4 based on severity. External piles form under the skin around the anal opening. Both types can cause bleeding, discomfort, itching and, in advanced cases, prolapse — where the pile protrudes outside the anus. Grade alone determines what treatment is appropriate — medication and diet, rubber band ligation, or laser surgery.
Most patients I see in the early stages are Grade 1 or 2 and can be managed with medication, dietary correction and lifestyle changes. Grade 3 and 4 typically need a procedure — and this is where laser hemorrhoidopexy gives the most consistent results in my hands.

How Do You Know If You Have Piles?
Rectal Bleeding
Bright red blood on toilet paper or in the toilet bowl after passing stool. Usually painless in internal piles.
Lump Near Anus
A soft swelling or prolapsed tissue near or at the anal opening. May be pushed back manually in Grade 3.
Itching & Wetness
Persistent itching, moisture or mucus discharge around the anal area from internal piles.
Heaviness
Feeling of incomplete evacuation or pressure in the rectum even after passing stool.
Pain on Straining
Discomfort during or after bowel movement. Severe sharp pain usually indicates a thrombosed pile or fissure.
Bleeding with Anaemia
Chronic painless bleeding from Grade 2–3 piles can lead to iron deficiency anaemia over time.
Piles Classification
Understanding the 4 Grades of Piles
Grade 1
Internal Bleeding Only
Piles remain inside the anal canal. Only symptom is painless bleeding. Typically managed with medication and diet correction — no surgery needed.
Grade 2
Prolapse & Return
Piles prolapse during straining but return inside on their own. Rubber band ligation, injection therapy, or laser hemorrhoidopexy all work well at this stage.
Grade 3
Manual Reduction
Prolapses and must be pushed back manually. In my experience, laser hemorrhoidopexy is the most reliable treatment at this stage — minimal pain, quick recovery.
Grade 4
Permanent Prolapse
Permanently prolapsed, cannot be reduced. Requires surgical intervention — laser hemorrhoidopexy or, in select cases, conventional surgery.

I began my surgical career with open haemorrhoidectomy — the conventional technique that has been the standard in Pakistan for decades. I watched patients struggle for weeks with painful wounds, time off work, fear of using the toilet, and a recurrence rate that I personally felt was too high.
