In my clinical experience, the patients who do best with Grade 1 piles are those who take lifestyle seriously. Increase fibre to 25 to 35 grams per day — psyllium husk (ispaghol) is widely available and very effective in Pakistan. Drink 8 to 10 glasses of water daily. Exercise moderately — even a 20-minute walk helps bowel regularity. Avoid sitting on the toilet for more than 2 to 3 minutes. Avoid straining.

I recommend a procedure — typically sclerotherapy — when: bleeding has been occurring for more than 6 weeks despite dietary changes, bleeding is frequent enough to cause anaemia, or symptoms significantly impact quality of life (daily bleeding, constant awareness).
