The most common cause is a perianal abscess — a collection of pus that forms when a small gland inside the anal canal becomes blocked and infected. In my experience, the single most common mistake patients make is delaying drainage because they are hoping antibiotics will resolve it. What Happens If a Perianal Abscess Is Left Without Anal Abscess Drainage?

Without drainage, several things can happen — none of them good. The abscess may spontaneously rupture through the skin, providing temporary relief but leaving an uncontrolled wound. More worryingly, the infection can spread to the surrounding tissue. In rare but serious cases, a condition called Fournier’s gangrene can develop — a necrotising infection of the perineum that is life-threatening.
