What Is Causing Painful Swelling Near the Tailbone — Is It Piles or Something Else?
Piles are located in the anal canal — inside or just at the anal opening. The tailbone (coccyx) is several centimetres above the anus. A swelling specifically near the tailbone — in the crease between the buttocks — is almost never piles. It is much more likely to be a pilonidal sinus or cyst.
A pilonidal sinus is a small tunnel or cavity in the skin, typically occurring in the natal cleft just above the anus. It usually involves ingrown hairs and skin debris, and it can become infected, causing a painful swollen lump. In young men especially — particularly those who sit for long hours or have coarse hair — this is very common.
What Are the Key Symptoms That Distinguish Pilonidal Sinus From Piles?

The typical patient is someone who sits for long hours — students, IT professionals, drivers. People with thick, coarse body hair are also at higher risk because the hair tends to penetrate the skin more easily in the natal cleft.
