How Does Banding Compare to Laser for Grade 2 Piles?

My usual approach: offer banding first for Grade 2. If the patient wants a more definitive one-time solution — or if piles recur after banding — we discuss laser. Both are valid. What Signs Indicate That Grade 2 Piles Need More Than Just Banding?

I escalate beyond banding when: the patient has had two or more courses of banding with recurrence within 12 months, there is a large external haemorrhoidal component alongside the internal pile, bleeding is severe enough to cause anaemia, or the patient has significant thrombosis or skin tags requiring surgical excision.
