The reasons are straightforward: no anaesthesia required, done entirely in clinic in 5 to 10 minutes, no incision, immediate return to normal activity, and success rate of 80 to 90 percent for Grade 2 in the short to medium term.

The procedure places an elastic band at the pile’s base, cutting off blood supply. The pile shrinks over 7 to 10 days and falls off during a bowel movement. The patient usually goes home immediately and returns to work the same day or next day. My usual approach: offer banding first for Grade 2. If the patient wants a more definitive one-time solution — or if piles recur after banding — we discuss laser. Both are valid. What Signs Indicate That Grade 2 Piles Need More Than Just Banding?

I escalate beyond banding when: the patient has had two or more courses of banding with recurrence within 12 months, there is a large external haemorrhoidal component alongside the internal pile, bleeding is severe enough to cause anaemia, or the patient has significant thrombosis or skin tags requiring surgical excision.

In these cases, laser haemorrhoidoplasty gives a more definitive and durable result. I prefer laser over open surgery for Grade 2 — the recovery is dramatically faster and outcomes are equivalent.
