What Causes a Red, Hot, Painful Lump Near the Anus?

The classic presentation I am describing — red, hot, swollen, tender to touch — is a perianal abscess. This is a collection of pus that forms in the tissues surrounding the anus. It starts as an infection in one of the small anal glands, which then spreads into the surrounding fat tissue and forms this acutely painful lump.

Piles, in contrast, are swollen blood vessels. They are soft, not hot. They may be uncomfortable but they are not typically warm to touch or associated with fever. The redness, heat and progressive swelling over hours to days is what distinguishes an abscess from piles.

What Is the Correct Treatment for Perianal Abscess?

The only effective treatment for a perianal abscess is drainage. Antibiotics alone do not cure an abscess — they can help control the surrounding infection, but the pus collection needs to be surgically drained. The procedure is done under local or general anaesthesia, a small incision is made, the pus is drained, and the cavity is cleaned.

Pain relief is almost immediate after drainage. I have had patients who could barely sit come in for the procedure and walk out with dramatically reduced pain within an hour. It is one of the most satisfying results to see as a surgeon because the improvement is instant and significant.

What Happens If a Perianal Abscess Is Not Treated Promptly?

In my experience, this is where delay becomes very costly. An untreated perianal abscess can do two things: spread the infection into deeper tissue spaces — which can become a surgical emergency called Fournier’s gangrene — or it can rupture spontaneously and leave behind a fistula tract that requires a separate procedure later. Should You Go to Emergency for a Red, Hot Anal Lump?

Yes — if you have fever above 38.5°C along with the swelling, if the swelling is spreading rapidly, if you cannot sit or walk due to pain, or if you have diabetes or any immune compromise. These are situations where the infection can spread quickly and you need same-day care.

How Can You Reduce the Risk of Getting a Perianal Abscess in the First Place?

Perianal abscesses usually start from an infected anal gland. While some people are simply more prone to this, there are practical steps that reduce the risk. Keep the anal area clean and dry — but avoid harsh soaps that irritate the skin. Treat constipation and diarrhoea promptly because both increase the stress on anal glands. People with diabetes need to be especially careful because their immune response is slower and infections spread faster. If you have had an abscess before, the follow-up is crucial — to check whether a fistula has formed. Recurrent abscesses in the same area almost always mean there is an underlying fistula that has not been treated. Address the fistula, and the abscesses stop.
